Join Indiana University Health's Community Benefit team in Bloomington as an RN supporting the GROW Initiative, providing comprehensive maternal and infant health services through assessment, education, care coordination, outreach, and referral management for pregnant and postpartum individuals, infants, and women of childbearing age. The RN works in both clinical and community settings, meeting clients where they are to improve engagement and reduce transportation barriers, with services delivered in healthcare facilities, community locations, homes, and other approved settings throughout Brown and Lawrence Counties. This Rural Health Transformation Program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $206,927,896.80 with 100 percent funded by CMS/HHS. Requirements
- Minimum of one year of RN OBGYN experience.
- Valid driver's license.
- Reliable transportation.
- Ability to travel throughout the service area.
- Strong communication skills.
- Strong assessment skills.
- Strong care coordination skills.
- Strong patient education skills.
- Requires an Associates of Nursing (ASN).
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Requires that the RN has graduated from a nationally accredited nursing program.
- Requires Basic Life Support certification through the AHA.
Nice To Haves
- Experience in OB, maternal-child health, labor and delivery, postpartum, women's health, pediatrics, community health, community outreach, care coordination, patient navigation, and working with vulnerable or underserved populations.
- Knowledge of maternal health resources, community programs, and social service systems.
- Bachelors of Nursing (BSN) preferred.
- Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
Responsibilities
- Providing comprehensive maternal and infant health services through assessment, education, care coordination, outreach, and referral management for pregnant and postpartum individuals, infants, and women of childbearing age.
- Working in both clinical and community settings, meeting clients where they are to improve engagement and reduce transportation barriers.
- Delivering services in healthcare facilities, community locations, homes, and other approved settings throughout Brown and Lawrence Counties.
