About The Position
We are seeking an experienced RN for a Long-Term Care facility supporting rehabilitation, Alzheimer's, and dementia residents. The ideal candidate will have LTC experience and strong medication administration and resident care skills.
Requirements
- Active New York RN License required.
- BLS Certification required.
- Experience with medication administration and resident assessments.
- Strong communication and documentation skills.
Nice To Haves
- Minimum 1 year of Long-Term Care (LTC) experience preferred.
- Acute Rehab, Med-Surg, or Acute Care experience considered.
Responsibilities
- Provide nursing care to LTC and rehabilitation residents.
- Administer medications and treatments as prescribed.
- Monitor resident conditions and report changes.
- Supervise and support CNAs on assigned units.
- Maintain accurate documentation in the EMR system.
- Ensure compliance with facility policies and patient safety standards.
- Collaborate with physicians and interdisciplinary teams.
