About The Position

Elite Care Management is hiring a Registered Nurse or Licensed Practical Nurse to provide one-on-one home health care to a patient in Riverwoods. The nurse works with one patient for the complete shift, getting to know the patient and family and helping plan, organize, and provide care in the home. The role includes assisting with daily living activities and supporting the patient's comfort, well-being, and quality of life. The listed part-time schedule is 7 a.m. to 7 p.m. on Tuesdays and Thursdays. Requirements

Driver's license.
Social Security card.
Auto insurance card.
Current CPR card.
Current state RN or LPN license.
TB test results: a two-step test or chest X-ray within the last 12 months, or QuantiFERON Gold blood work within the last five years.

Responsibilities

Observe the patient, record significant conditions and reactions, and notify a supervisor or physician of the patient's condition, reactions to drugs or treatments, and significant incidents.
Follow organizational and nursing policies and procedures.
Communicate effectively in spoken and written form.
Manage and direct patient care according to the patient care plan, updating the plan as needed.
Administer prescribed medications and treatments and monitor and document their effectiveness.
Respond as appropriate to changes in the patient's condition and report those changes to the appropriate physician or supervisor.
Assist the patient with daily living activities and support the patient's comfort and overall well-being.
