About The Position

This role focuses on enhancing communication, care coordination, barrier removal, reducing readmissions, and providing emotional support to patients and families within the Critical Care department at Ascension St. John Medical Center. The Registered Nurse Patient Navigator will serve as a central point of contact, ensuring clear understanding of diagnoses and treatment plans, facilitating smooth transitions of care, addressing obstacles to care, and offering psychosocial support during critical illness.

Requirements

Licensed Registered Nurse obtained prior to hire date or job transfer date required.
Registered Nurse credentialed from the Oklahoma Board of Nursing required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
5+ years of Critical Care nursing experience, preferably in an ICU setting.

Nice To Haves

BLS Provider preferred. American Heart Association or American Red Cross accepted.

Responsibilities

Serve as a central point of contact, ensuring that patients and families clearly understand complex diagnoses and treatment plans.
Facilitate seamless transitions from the ICU to step-down units or to home, reducing treatment delays and ensuring continuity of care.
Identify and address obstacles such as financial, transportation, or social issues to ensure timely access to necessary services.
Provide follow-up support and education on self-management to decrease emergency room visits and urgent readmissions.
Provide psychosocial support to patients and their families, helping them navigate the distress associated with critical illness.
