About The Position

Haven Hospice is seeking a passionate and dedicated Hospice Admissions RN to join their team in Palatka, FL. Requirements

• Graduate of an accredited nursing school with current licensure to practice in the state of operation.
• One year of hospice nursing experience, or 3+ years in a similar setting such as Long-Term Care, Intensive Care, or Emergency Room.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Subject to a level 2 background screening.

Nice To Haves

• 3+ years of hospice nursing experience preferred.
• Experience in Long-Term Care, Intensive Care, or Emergency Room will be considered in lieu of hospice nursing experience.

Responsibilities

• Visit patients in homes, hospitals, and nursing facilities who are referred for hospice care.
• Explain hospice services, discuss goals of care with patients and families, answer questions, and obtain necessary consents and legal documents.
• Obtain medical history, current medications, and symptom information.
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines.
• Coordinate the initiation of services, including nursing, aides, equipment, and medications.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
