About The Position

Elara Caring is seeking a Registered Nurse Home Health to provide compassionate care to patients in their homes. The nurse assesses patients’ care needs, completes initial and ongoing assessments, and provides nursing care in accordance with each patient’s plan of care and physician orders. The role includes monitoring patient conditions, reporting changes to the care team, educating patients and families about care and safety, protecting privacy under HIPAA, and taking appropriate action when a patient’s condition changes. The nurse helps ensure that care is quality, safe, and continuous.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.
