About The Position

Elara Caring is seeking a compassionate Registered Nurse Home Health to provide care in patients’ homes. The nurse assesses patient needs, completes initial and ongoing assessments, and provides nursing care according to the plan of care and physician orders. The role includes monitoring patient conditions, reporting changes to the care team, educating patients and families about care and safety, and responding appropriately when conditions change. The nurse also helps ensure quality, safe, and continuous care while protecting patient and staff privacy in accordance with HIPAA.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.
