About The Position

CommonSpirit Health at Home is hiring a PRN Home Health Registered Nurse in Indianapolis, Indiana. Requirements

• Graduate of an accredited nursing program.
• Active RN license in the state served.
• At least 1 year of nursing experience.
• Exceptional critical-thinking skills, proven clinical expertise, strong case-management abilities, and commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver high-quality, personalized nursing care in patients’ homes.
• Develop, implement, and adjust comprehensive care plans to promote patient independence and well-being.
• Coordinate care with physicians, therapists, and support staff.
• Educate patients and families and provide emotional support.
• Oversee the caseload and guide LPNs, LVNs, and Home Health Aides.
• Use technology and best practices to optimize care and maintain accurate documentation.
