About The Position
CommonSpirit Health at Home is hiring a Home Health Registered Nurse in Indianapolis, Indiana. The role delivers personalized nursing care in patients’ homes, with a 1:1 patient-clinician ratio and time to educate, empower, and support patients throughout their recovery. Nurses guide patients from initial visits through recovery, adapting care plans and celebrating milestones. The position offers career development opportunities, including potential paths in clinical leadership, operations, and education. It uses NestMed ambient AI technology and does not require after-hours on-call work.
Requirements
- Graduate of an accredited nursing program.
- Active RN license in the state served.
- At least 1 year of nursing experience.
- Exceptional critical-thinking skills and proven clinical expertise.
- Strong case-management abilities and commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.
Nice To Haves
- Home Health experience is a significant plus.
Responsibilities
- Deliver high-quality, personalized nursing care in patients’ homes.
- Develop, implement, and adjust comprehensive individualized care plans to promote patient independence and well-being.
- Coordinate care with physicians, therapists, and support staff.
- Educate and support patients and their families, including providing emotional support.
- Oversee a caseload and guide LPNs, LVNs, and Home Health Aides.
- Use technology and best practices to optimize care delivery and maintain accurate documentation.
