About The Position

AccentCare is hiring a full-time Registered Nurse, Home Health Case Manager based at its Marysville, Pennsylvania office. The position is not remote and covers York County, Pennsylvania, and surrounding areas. The RN Case Manager manages patients’ care plans from admission through discharge and ensures quality patient care. The role combines direct and indirect patient care, directs staff nurses, LPNs, and CHHAs in delivering each patient’s plan of care, identifies interdisciplinary needs, and coordinates allied health clinicians. Compensation is estimated at $91,000–$101,000 per year and is paid on a per-visit basis. A $10,000 bonus is offered. The company also sponsors specialty certifications and credentials, including GCS, HCS-O, LSVT BIG, and CAPS.

Requirements

• Graduate from an approved school of professional nursing.
• Hold a current license to practice as a registered nurse in the state where the agency operates.
• Have at least one year of experience as an RN.
• Hold current CPR certification.

Responsibilities

• Assess the nursing needs of assigned patients and manage their overall care plans.
• Complete an initial patient assessment upon admission, including obtaining an appropriate patient history.
• Select appropriate nursing diagnoses.
• Assess medications for ineffective therapy, adverse reactions, significant side effects, allergies, and contraindications.
• Establish realistic, measurable, observable goals consistent with each patient’s diagnosis.
• Inform the physician of assessment results and recommended clinical interventions.
• Implement nursing interventions consistent with the patient’s diagnosis, established goals, and the scope of the Nurse Practice Act.
• Re-evaluate patients’ nursing and other clinical needs as required.
• Document and submit assessments, progress notes, discharge notes, and other required paperwork in a timely manner.
• Inform the physician and other personnel of changes in a patient’s condition and needs.
• Direct staff nurses, LPNs, and CHHAs in delivering each patient’s plan of care; identify interdisciplinary needs and coordinate allied health clinicians.
