About The Position

Elara Caring is seeking a Registered Nurse Home Health to provide compassionate nursing care to patients in their homes. This position supports Chicago and the western suburbs. The nurse assesses patient needs, provides care according to plans of care and physician orders, monitors and reports changes in patient condition, and educates patients and families. The role offers a $7,500 sign-on bonus. The stated base salary range is $83,000 to $93,000 annually; actual pay depends on factors including education, experience, skills, and internal equity.

Requirements

• Hold a current state license as a Registered Nurse.
• Graduate from an accredited certificate, diploma, associate, or baccalaureate school of nursing.
• Have at least 1 year of experience as a nurse in a clinical care setting.
• Be able to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.

Responsibilities

• Assess patients to determine their care needs.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy in accordance with HIPAA.
• Provide nursing care according to the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families about their care and safety.
• Take appropriate action when patient conditions change.
• Help ensure quality, safe, and continuous patient care.
