About The Position

Discovery at Home is hiring a PRN Registered Nurse for home health visits. The role requires being on call, and pay varies by visit type: Start of Care $125; Recertification $80; Resumption of Care $80; OASIS Follow-Up (SCIC) $80; OASIS Discharge $60; Routine Visit $55; Psych Start of Care $125; Psych Visit $55; and MSW $120.

Discovery at Home is part of Discovery Senior Living and provides Part A, Part B, and Private Duty services. The organization began with a Medicare-certified agency in Southwest Florida in 2012 and has expanded to locations in Tampa, Florida; Plano and Houston, Texas; and West Palm Beach, Florida. Its services primarily support residents of Discovery Senior Living communities, while also serving people beyond those communities.

The nurse provides comprehensive home health nursing care, assesses patients and develops individualized care plans, administers prescribed treatments, monitors and documents progress, coordinates care with interdisciplinary team members, and educates patients and families. The role also includes responding to emergencies, maintaining accurate records, following infection control protocols, and keeping nursing knowledge current through training and continuing education.

Florida employment is contingent on completing a Level 2 background screening, including fingerprinting, through the AHCA Clearinghouse. Discovery at Home states that it is an equal opportunity employer.

Requirements

• Must be licensed or registered by the applicable state without restrictions.
• Bachelor’s degree in nursing (BSN) or Associate degree in Nursing (ADN) from an accredited nursing program.
• One year of home health experience required.
• Knowledge of Medicare and Medicaid guidelines.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skills and ability to use Microsoft Windows.
• Florida employment requires completion of a Level 2 background screening, including fingerprinting, through the AHCA Clearinghouse.

Responsibilities

• Provide comprehensive nursing care in accordance with professional nursing standards, policies, and procedures.
• Assess patient needs and develop and implement individualized care plans.
• Administer medications, treatments, and therapies as prescribed by healthcare providers.
• Monitor and document patient progress, including vital signs, symptoms, and changes in medical condition.
• Collaborate with interdisciplinary healthcare team members to coordinate and provide continuity of care.
• Educate patients and families about treatment plans, medications, and self-care techniques.
• Respond promptly and accurately to emergencies and perform life-saving interventions.
• Maintain accurate, up-to-date medical records, including health history, treatment plans, and progress.
• Follow infection control protocols and help maintain a safe, healthy environment for patients and staff.
• Keep current with nursing practices and attend training and continuing education.
