About The Position

Elara Caring is hiring a PRN Home Health Registered Nurse for the Bryan-College Station area. The role provides independent, one-on-one nursing care in patients’ homes, supporting their recovery and ability to remain independent. The nurse oversees care from assessment through discharge and is expected to provide consistent availability, including specifying the days and hours they can work each week.

Requirements

Current Registered Nurse license in the state of practice.
Graduation from an accredited nursing program.
At least one year of nursing experience in a Home Care setting.
Home health experience and familiarity with Homecare Homebase (HCHB).
Valid driver’s license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
Ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment.

Responsibilities

Complete comprehensive initial and ongoing patient assessments.
Develop individualized plans of care based on patient needs and physician orders.
Provide skilled nursing visits in patients’ homes.
Manage an assigned caseload and coordinate services across the care team.
Monitor patients’ physical, psychosocial, environmental, and caregiver needs.
Identify changes in condition and communicate appropriate interventions.
Educate patients and caregivers on medications, diagnoses, treatments, and disease management.
Collaborate with physicians, therapists, social workers, and clinical leaders.
Update care plans as patients’ needs and goals change.
Complete accurate and timely clinical documentation.
