Home Health Registered Nurse-PRN
About The Position
As a CenterWell Home Health nurse, reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence at home. You work with physicians and other clinicians to manage care plans that support recovery. The role provides skilled nursing care in patients’ homes, including medication administration, wound care, and patient assessments. You develop and implement individualized care plans, monitor patient conditions, educate patients and families, maintain care records, coordinate with other healthcare professionals, and report progress to the patient’s physician and Clinical Manager. You also oversee Home Health Aides and monitor completion of their documentation. This is a patient-facing role subject to Humana’s TB screening program. It is also part of the driver safety program and requires a valid state driver’s license and personal vehicle liability insurance meeting applicable minimum limits.
Requirements
- Med surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities
- Provide skilled nursing care in patients’ homes, including administering medications, performing wound care, and conducting patient assessments.

- Educate patients and their families on disease management, medication, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient’s physician and Clinical Manager.
- Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.
