About The Position

DaVita Kidney Care is launching a new team called DaVita Skilled Nursing Dialysis Services (DSD Services) to provide on-site dialysis treatments at skilled nursing facilities (SNFs). This initiative aims to reduce the burden on dialysis patients residing in SNFs and decrease healthcare costs. DSD Services teammates will leverage DaVita's resources to rapidly grow this new service line. The ideal candidate thrives in a start-up environment, is passionate about improving patient lives, and demonstrates a "Get Stuff Done" (GSD) attitude. Comfort with autonomy and cross-functional collaboration is essential. As a DSD RN, you will be responsible for the complete ongoing care of each patient, ensuring their dialysis treatment is on track. This includes troubleshooting, making sound decisions, and occasionally partnering with SNF staff for coordinated care. The role involves patient education, direct patient care, supervising Dialysis Technicians, and managing complex dialysis care in an independent setting. Critical thinking and continuous assessment are key to providing comprehensive care and improving patient outcomes.

Requirements

  • Current Registered Nurse (RN) license in the state of practice
  • Minimum of 12 months' of RN experience
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required
  • Basic computer skills and proficiency in MS Word and Outlook

Nice To Haves

  • Previous Chronic Hemodialysis, Acute Dialysis and/or Peritoneal Dialysis (PD) nursing experience
  • Training/Teaching/Healthcare Education background
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN)
  • Bachelor of Science in Nursing (B.S.N)
  • three-year diploma from accredited diploma program may be substituted for nursing degree

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
  • Troubleshoot and make sound decisions for patients related to their dialysis treatment.
  • Occasionally partner alongside the SNF staff to coordinate care.
  • Conduct impactful and meaningful education to patients and their families in the SNF setting.
  • Independently demonstrate best practice techniques and deliver optimal dialysis care.
  • Supervise Dialysis Technicians who are tasked with delivering tandem care to meet the capacity and efficiency goals of the program.
  • Meet and/or exceed clinical outcomes.
  • Multitask to meet the needs of the service, the patients, and SNF partners.
  • Provide comprehensive patient care in an independent setting.
  • Assess, diagnose and implement complete care plans by looking at the whole patient.
