About The Position

DaVita Kidney Care is expanding its services to patients in skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services (DSD). This initiative aims to provide clinical quality dialysis care on-site at SNFs, reducing the burden on patients who are currently transported for treatment three times a week. The DSD Services team will leverage DaVita's resources to develop and grow this new service line. Teammates in this role should be experienced, comfortable working autonomously, and able to collaborate with cross-functional teams. The role involves direct patient care, ensuring safe, comfortable, and hygienic dialysis treatments. DSD Dialysis care is complex, requiring multitasking and a focus on meeting clinical outcomes. PCTs will monitor patients before, during, and after dialysis, including measuring stats, observing patients, and setting up hemodialysis machines. Training will be provided through DaVita's award-winning programs, which may include classroom and hands-on learning in a facility or training clinic. Hours may vary during training, typically 32-40 hours per week. Teammates may also be asked to support other local SNF dens.

Requirements

  • Prior Hemo-Dialysis experience
  • Valid Certified Clinical Hemodialysis Technician (CCHT) certification
  • High school diploma or equivalent
  • Basic computer skills and proficiency in MS Word and Outlook

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
  • Work in conjunction with dialysis RNs to provide dialysis care.
  • Provide safe, comfortable and hygienic dialysis treatment.
  • Monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup.
  • Support other local SNF dens as needed.
