About The Position

DaVita Kidney Care is expanding its services by bringing industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. This initiative aims to address the significant burden on dialysis patients residing in SNFs who are transported for treatment three times a week, as well as reduce avoidable healthcare costs. DSD Services teammates will leverage DaVita's resources to develop and grow this new service line. The ideal candidate will have experience in dialysis, thrive in a start-up environment, be passionate about improving patient lives, and possess a proven ability to 'Get Stuff Done' (GSD). Comfort with autonomous work and collaboration with cross-functional teams is essential. In this role, you will be responsible for the complete ongoing care of each patient, ensuring their dialysis treatment is on track. DSD RNs will troubleshoot and make critical decisions regarding patient dialysis treatments, occasionally collaborating with SNF staff for coordinated care. The primary work schedule is three days a week, with potential for occasional Saturday work. No on-call duties are required. Your work will directly impact the lives of patients and their families daily. You will have the opportunity to provide impactful education to patients and their families in the SNF setting. The RN's primary responsibility is to independently deliver optimal dialysis care using best practice techniques, including supervising Dialysis Technicians. DSD Dialysis care is a complex but fulfilling service line where meeting and exceeding clinical outcomes is the goal, and multitasking is expected to meet the needs of the service, patients, and SNF partners. You will provide comprehensive patient care in an independent setting, requiring constant critical thinking and application of core nursing skills to assess, diagnose, and implement complete care plans for the whole patient. The role involves significant tracking and management, with the reward of witnessing the positive impact of your care on patients' lives.

Requirements

  • Current Registered Nurse (RN) license in the state of practice
  • Minimum of 12 months' of RN experience
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required
  • Basic computer skills and proficiency in MS Word and Outlook

Nice To Haves

  • Previous Chronic Hemodialysis, Acute Dialysis and/or Peritoneal Dialysis (PD) nursing experience
  • Training/Teaching/Healthcare Education background
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN)
  • Bachelor of Science in Nursing (B.S.N)
  • three-year diploma from accredited diploma program may be substituted for nursing degree

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
  • Troubleshoot and make sound decisions for patients related to their dialysis treatment.
  • Occasionally partner alongside the SNF staff to coordinate care.
  • Conduct impactful and meaningful education to patients and their families in the SNF setting.
  • Independently demonstrate best practice techniques and deliver optimal dialysis care.
  • Supervise Dialysis Technicians who are tasked with delivering tandem care to meet the capacity and efficiency goals of the program.
  • Provide comprehensive patient care in an independent setting.
  • Assess, diagnose and implement complete care plans as you look at the whole patient.
