About The Position
Spectrum Healthcare Resources has a potential opportunity for a dedicated Registered Nurse – Disease Manager supporting chronic disease management and preventive healthcare at Joint Base Elmendorf-Richardson, Alaska. The role helps patients improve their health, manage chronic conditions, and achieve better long-term outcomes. This is a full-time position, Monday through Friday, from 7:30 AM to 4:30 PM.
Requirements
- U.S. citizenship.
- Bachelor of Science in Nursing (BSN) from an ACEN- or CCNE-accredited program.
- Current, unrestricted RN license from any U.S. jurisdiction.
- Two years of full-time Disease Management and Care Coordination experience within the last 3–4 years.
- Experience managing chronic conditions such as diabetes, hypertension, COPD, or heart failure.
- Current BLS certification.
- Strong patient education and care coordination skills.
Responsibilities
- Develop and monitor chronic disease management plans.
- Educate patients on disease prevention and self-management.
- Coordinate care with providers and multidisciplinary teams.
- Track patient progress and support improved health outcomes.
