Nurse Disease Management I
About The Position
The Nurse Disease Management I participates in delivering patient education and disease management interventions and provides health coaching to members across multiple lines of business to support health improvement and management of chronic diseases. The position conducts behavioral or clinical assessments, develops and coordinates coaching plans, collaborates with providers and other health professionals, and monitors the effectiveness of interventions. The role is primarily virtual, with required in-person training sessions. Candidates must live within a reasonable commuting distance of the posting location unless an accommodation is granted as required by law.
Requirements
- Associate of Science degree in nursing and at least 2 years of condition-specific clinical or home health/discharge planning experience, or an equivalent combination of education and experience.
- Current unrestricted RN license in applicable state(s).
Nice To Haves
- Bachelor of Science degree in nursing preferred.
- Prior case management experience preferred.
- Labor and Delivery, Postpartum, or OB Clinic experience strongly preferred.
Responsibilities
- Conduct behavioral or clinical assessments to identify members’ knowledge, skills, and behavioral needs.
- Identify or coordinate health coaching plans to address objectives and goals established during assessments.
- Interface with providers and other health professionals to coordinate members’ health coaching plans.
- Implement or coordinate coaching and care plans by educating members about clinical needs and facilitating referrals for behavioral health needs.
- Use motivational interviewing to facilitate health behavior change.
- Monitor and evaluate the effectiveness of interventions and health coaching plans, modifying them as needed.
- Direct members to facilities, community agencies, and appropriate providers or networks.
- Refer members to catastrophic case management.
