About The Position

The Nurse Disease Management I participates in delivering patient education and disease management interventions and provides health coaching to members across multiple lines to support health improvement and management of chronic diseases. The role conducts assessments, develops and coordinates coaching plans, works with providers and other health professionals, and helps members access appropriate care and community resources. The position is primarily virtual, with required in-person training sessions. Candidates must live within a reasonable commuting distance of the posting location unless an accommodation is granted as required by law. The role is classified as non-management and exempt.

Requirements

Associate degree in nursing and at least 2 years of condition-specific clinical experience or home health/discharge planning experience, or an equivalent combination of education and experience.
Current unrestricted RN license in applicable state(s).

Nice To Haves

Bachelor of Science in nursing.
Prior case management experience.
Labor and Delivery, Postpartum, or OB Clinic experience.

Responsibilities

Conduct behavioral or clinical assessments to identify members’ knowledge, skills, and behavioral needs.
Identify or coordinate health coaching plans that address objectives and goals found through assessments.
Interface with providers and other health professionals to coordinate members’ health coaching plans.
Implement or coordinate coaching and care plans by educating members about clinical needs and facilitating referrals for behavioral health needs.
Use motivational interviewing to facilitate health behavior change.
Monitor and evaluate intervention and health coaching plan effectiveness, modifying plans as needed.
Direct members to facilities, community agencies, and appropriate providers or networks.
Refer members to catastrophic case management.
