This role is ideal for a nurse who enjoys one-on-one care, field visits, documentation accuracy, and helping ensure clients receive safe, personalized care. Requirements

  • Active Registered Nurse (RN) license in Florida
  • Private-duty home care experience preferred
  • Strong understanding of home care regulations, documentation standards, and client care planning
  • Experience completing assessments, reassessments, supervisory visits, or field visits preferred
  • Excellent communication, leadership, and problem-solving skills
  • Ability to work independently and manage multiple priorities
  • Comfortable using electronic documentation systems and AI; WellSky experience preferred
  • Valid driver’s license, auto insurance, and reliable transportation for field visits
  • CPR certified, or willing to complete
  • TB test within the last 6 months, or willing to complete
  • Level 2 AHCA background screening, or willing to complete

Responsibilities

  • Conduct home visits to complete client assessments, reassessments, care plan reviews, and progress evaluations
  • Complete caregiver supervisory visits to observe care delivery, ensure care plan compliance, and provide coaching or guidance as needed
  • Review and update individualized client care plans based on client needs, condition changes, and family or care team feedback
  • Ensure compliance with state home care regulations, agency policies, and documentation standards
  • Provide training, mentorship, and clinical guidance to caregiving staff to promote quality care and professional growth
  • Communicate effectively with clients, families, caregivers, physicians, and interdisciplinary team members to support positive care outcomes
  • Maintain accurate, timely, and complete clinical documentation in accordance with agency protocols within the WellSky platform
  • Report directly to the Director of Nursing regarding care quality, client concerns, caregiver performance, and compliance matters
  • Conduct medication box pours and ensure medication management procedures are accurately followed
  • Identify safety concerns, changes in client condition, or care plan needs and communicate appropriately with the Director of Nursing and care team
  • Support quality assurance efforts, chart reviews, and compliance readiness as needed
  • Rotating On-Call with the Scheduler on-call; separate pay incentive of $400 for the week
