About The Position

Continuum Hospice is seeking a full-time Hospice Registered Nurse Case Manager (RNCM) to provide compassionate end-of-life care to patients and families throughout Alameda and Contra Costa Counties. The role combines hands-on hospice nursing, care coordination, and guidance for patients and families throughout the hospice journey. As part of an interdisciplinary care team, the RNCM helps provide care focused on compassion, clinical excellence, dignity, comfort, and human connection. The position is scheduled Monday through Friday, 8:00 a.m. to 4:30 p.m., and shares responsibility for call duties.

Requirements

Hold a current, unencumbered Registered Nurse license.
Have a bachelor’s degree in nursing from a program accredited by the National League for Nursing, or have one year of experience as a professional nurse within the last three years.
Have at least one year of hospice experience.
Be proficient in typing and computer programs, including electronic medical records (EMR).
Maintain current CPR certification.
Have a current driver’s license, vehicle insurance, and access to dependable transportation.

Responsibilities

Respond compassionately and expertly to clinical referral information, inquiries, and requests.
Work with the Clinical Director to assess client eligibility and suitability for hospice services.
Provide direct nursing care and support patients’ comfort and quality of life.
Deliver nursing care using established principles and techniques to help ease pain and provide comfort.
Educate and advocate for patients and families, encouraging their participation in personalized care planning.
Share responsibility for call duties.
