About The Position
Baptist Health is seeking a Registered Nurse, Case Manager to join the In-Patient Case Management Department at Baptist Medical Jacksonville - Downtown campus. This is a Full Time Day shift opportunity, working Monday - Friday. Requirements
- 1 year Case Management experience
- Associate's Degree Nursing (ADN)
- Basic Life Support (BLS)
- Driver's Licensed (DMV)
- FL Active licensed Registered Nurse (RN)
Responsibilities
- Care coordination for hospitalized patients.
- High risk screenings and assessments.
- Coordinating post-acute care services with proper documentation.
- Collaborating with multidisciplinary team members.
