About The Position

Haven Hospice is seeking a Registered Nurse Case Manager for its Lake City, Florida office. The role provides hospice care across a five-county area of approximately 2,800 square miles. The nurse delivers patient-centered care in the home, supports patients and families, and works both independently and with an interdisciplinary team. The schedule is full-time, Monday through Friday, 8 a.m. to 5 p.m., with no weekends or holidays. Participation in an on-call rotation is required as determined by the local branch. The position is subject to a Level 2 background screening.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least one year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Licensed driver with an automobile insured according to state or organizational requirements and in good working order.
• Current CPR certification in states where required.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create hospice care plans aligned with patients’ wishes and goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the on-call rotation as required by the local branch.
