About The Position

Hospice Home Care is seeking a dedicated Registered Nurse Case Manager to provide patient-centered hospice care to patients and support to their families in Monticello, Arkansas. The role focuses on managing pain and other symptoms while providing emotional and spiritual support as part of a holistic, interdisciplinary care approach. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with participation in an on-call rotation as required by the local branch. The position offers independent work alongside team support and flexible scheduling options.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least one year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Licensed driver with an automobile insured in accordance with state or organization requirements and in good working order.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create hospice care plans aligned with each patient’s wishes and goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the on-call rotation as required by the local branch.
