About The Position

DaVita Integrated Kidney Care is hiring a Registered Nurse Case Manager to coordinate and facilitate quality, cost-effective care for patients with end-stage renal disease (ESRD) and chronic kidney disease (CKD), while minimizing fragmentation in the healthcare delivery system. The role coordinates care among patients, families, physicians, providers, care teams, community partners, and the Regional Operations Manager. The RN Case Manager works autonomously and collaboratively with the healthcare team. The position is hybrid/remote and field-based, covering Kissimmee and surrounding areas. It requires a dedicated home office and travel to meet business and patient needs, including nephrology partner meetings. The RN Case Manager is expected to work in the field three to four days per week in an assigned territory; territory assignments may change based on business needs. The schedule is Monday through Friday, with flexibility needed to accommodate patient needs and nephrology partner availability. DaVita Integrated Kidney Care is the integrated care division of DaVita Inc. Its team of more than 600 nurses, care coordinators, nurse practitioners, and business professionals manages care for more than 20,000 people with late-stage CKD and ESRD across the United States each month. The organization aims to improve patient quality of life and outcomes while lowering total cost of care, and offers training, growth, and development opportunities.
