About The Position

The RN Case Manager functions autonomously and in collaboration with the healthcare team to coordinate and facilitate quality, cost-effective care for patients with end-stage renal disease (ESRD) and chronic kidney disease (CKD), while minimizing fragmentation of care. The role coordinates care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager. This hybrid, remote, field-based position covers Kissimmee and surrounding areas. The RN Case Manager works in the field 3–4 days per week, including travel to meet patient and business needs and attend nephrology partner meetings. A dedicated home office with internet connectivity is required. Territory assignments may change according to business needs. The schedule is Monday through Friday, with flexibility to accommodate patient needs and nephrology partner availability. DaVita Integrated Kidney Care supports patients with late-stage CKD and ESRD through coordinated care management. Requirements

Associate Degree in Nursing (A.D.N.) from an accredited school of nursing; a three-year diploma from an accredited diploma program may be substituted for a nursing degree.
Current Registered Nurse (RN) license in the state of practice; the posting also states the candidate must hold an active Florida RN license or be able to obtain licensure in the state where not currently licensed.
Maintain continuing education credits as required by the state of practice.
At least five years of clinical nursing experience.
Demonstrated knowledge and understanding of data and management of clinical, financial, and patient satisfaction outcomes.
Demonstrated experience and effectiveness in a change-agent role.
Demonstrated knowledge and understanding of CQI techniques.
Previous experience in healthcare performance coaching.
Current CPR certification.
Ability to modify personal practice patterns to adapt to new or electronic processes and increased productivity expectations related to Capella implementation.
Current driver’s license in the state employed, a positive driving record, and ability to meet insurance coverage requirements.
Basic computer skills and proficiency in MS Word and Outlook.
Functional proficiency with DaVita-specific clinical software, including Capella, within 90 days of employment.
Dedicated home office with internet connectivity of at least 1 MB upload and 1 MB download speed.
Ability to travel in the assigned territory 3–4 days per week and accommodate patient and business needs.

Nice To Haves

Bachelor of Science in Nursing (B.S.N.).
At least three years of renal nursing experience.
Case management experience, including assessing patient needs, developing and managing individualized care plans, coordinating services across interdisciplinary teams, and supporting patients through complex healthcare transitions.
Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) certification.
