About The Position

Adoration Health is hiring a Hospice Registered Nurse (RN) Case Manager serving Baltimore City, Maryland, with an office location in Baltimore. This full-time role is scheduled Monday through Friday, 8:00 a.m. to 5:00 p.m. The Case Manager provides direct patient care and coordinates with the interdisciplinary team to support patients and families and deliver an optimal care experience. Adoration Health, an affiliate of BrightSpring Health Services, provides compassionate services in patients’ homes and support for patients, families, and caregivers. Requirements

Graduate of an accredited nursing school.
Current licensure to practice in the state of operation.
At least 1 year of nursing experience.
Knowledge of the hospice philosophy of care.
Commitment to clinical and documentation excellence.

Nice To Haves

Three or more years of nursing experience preferred.
Experience in hospice or a similar setting preferred.

Responsibilities

Complete initial and ongoing comprehensive assessments of patients and families.
Initiate patient plans of care and reevaluate them as necessary.
Use health assessment data to determine nursing diagnoses.
Administer medications and treatments as prescribed by the physician in the plan of care.
Counsel patients and families to address nursing and related needs.
Complete and maintain accurate clinical notes.
Communicate with physicians and participate in interdisciplinary group meetings to address patients’ and families’ or caregivers’ medical and emotional needs.
Ensure compliance with state and federal legal and regulatory requirements.
Perform additional responsibilities as assigned.
