About The Position

Hospice of North Alabama is seeking a Registered Nurse Case Manager to provide patient-centered hospice care in the Cullman, Alabama office’s East Morgan County coverage area. The role is scheduled Monday through Friday, 8 a.m. to 5 p.m. Hospice nurses provide care in patients’ homes and support patients and families during a critical time in their lives. The position offers independent work with team support and flexible scheduling options.

The RN Case Manager assesses and monitors patients’ physical, emotional, and psychological needs; develops care plans that align with patients’ wishes and goals; and provides nursing care, including medication administration, treatments, interventions, and pain and symptom management. The nurse educates and supports family members and caregivers, collaborates with the interdisciplinary team, maintains accurate and timely documentation, and participates in the on-call rotation as required by the local branch.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least one year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Licensed driver with an automobile insured according to state or organization requirements and in good working order.
• Maintain current CPR certification in states where required.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.
