About The Position
PruittHealth Hospice is hiring a Registered Nurse Case Manager to provide hospice care to patients in their homes or care facilities. The role is on-call on weekends and focuses on maintaining continuity of patient care while following established policies, procedures, and individualized care plans. Requirements
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
Responsibilities
- Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
- Complete all documentation and records in a timely and accurate manner.
- Follow established policies and procedures related to the scope of practice.
- Maintain continuity of patient care in a hospice setting.
