About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Registered Nurse Case Manager to join their TMO/UM Team D. This full-time position operates on day shifts, 40 hours per week, from 8 am to 6:30 pm, with four 10-hour shifts. The role requires weekend work on Saturdays and Sundays, with incentive pay offered. The Registered Nurse Case Manager will collaborate with the healthcare team to coordinate patient and family interventions across the continuum of care, aiming to remove barriers and promote efficient resource utilization. This includes assisting patients and families in establishing goals, facilitating appropriate follow-up care, and referring patients to the correct level or site of care. The position also serves as an advocacy role for patients, families, and caregivers to ensure safe healthcare. Additionally, the role assists in developing and meeting key Pillar outcomes and system improvement goals, including financial, satisfaction, and clinical aspects, as the nursing component of the care coordination model.

Responsibilities

  • Assesses and identifies patients with complex clinical needs that require a coordinated plan of care.
  • Develops case management plan that includes strategies or alternative interventions required to attain optimal patient and family specific outcomes.
  • Analyzes progress on the plan of care, identifies variances, and intervenes utilizing process improvement methodologies to remove barriers and progress care.
  • Integrates ethical provisions in all areas of the practice.
  • Demonstrates leadership skills and acts as the key information and education resource for the interdisciplinary team as related to caring for complex needs and securing appropriate services.
