About The Position

National Church Residences Home Health division in Southern Ohio is a Home Health & Hospice agency seeking a Hospice RN Case Manager. This role is with an organization that is owned and operated by National Church Residences, the nation’s largest provider of affordable senior housing and health care services. The agency is looking for a candidate who will share in the vision to advance better living and care for seniors.

Requirements

Degree in nursing field as a registered nurse from an approved school of nursing.
Minimum of two years as an RN.

Nice To Haves

Home health/hospice experience preferred.

Responsibilities

Provide case management services as a Registered Nurse Case Manager for Hospice patients.
Serve patients within the Jackson, Vinton, Pike, and Ross Counties service area.
