Registered Nurse – Case Management, Inpatient

About The Position
Spectrum Healthcare Resources has a potential opportunity for an experienced Inpatient RN Case Manager supporting patient care at Joint Base Elmendorf-Richardson in Alaska. The role is part of a collaborative healthcare team focused on patient outcomes, care coordination, and successful transitions. The position is full-time, Monday through Friday, 40 hours per week.
Requirements
- U.S. citizenship.
- An Associate Degree in Nursing or higher from an ACEN- or CCNE-accredited program.
- A current, unrestricted RN license from any U.S. jurisdiction.
- Three years of RN Case Management experience within the last four years.
- Case Management certification, such as CCM, CDMS, CRRN, or another PWS-approved credential.
- Current BLS certification.
Responsibilities
- Coordinate inpatient treatment and discharge planning.
- Assess patient needs and develop individualized care plans.
- Collaborate with physicians, nurses, and social workers.
- Facilitate transitions to outpatient and community services.
- Support exceptional care and successful recovery for military patients.
