About The Position

The Registered Nurse Care Coordinator with UVA Health Family Medicine at PCC supports patients navigating complex care needs across specialties, services, and transitions. This salaried role serves a broad patient population—including obstetric, pediatric, adult, obesity medicine, substance use, and refugee populations—in Charlottesville and surrounding communities. The coordinator combines patient outreach, clinical review, discharge follow-up, resource navigation, and interdisciplinary collaboration to promote continuity and help patients understand their care plans and next steps. The position works closely with residents, social workers, pharmacists, specialty clinics, imaging services, and other care team members in a collaborative ambulatory environment. It offers hybrid flexibility and a predictable schedule without evening, weekend, or holiday hours. The role contributes to population health, patient engagement, and individualized care planning within a growing Family Medicine program and an academic health system. Applicants must have a Bachelor of Science in Nursing from an accredited nursing program, at least one year of registered nursing experience, Virginia RN licensure, and current American Heart Association Health Care Provider BLS certification. Three years of related and relevant RN experience and specialty board certification are strongly preferred. Ambulatory care coordination and Epic experience are especially valuable. The annual pay range is $88,212.80–$115,793.60. Individual compensation depends on qualifications, prior work experience, and/or education. Sign-on bonus availability is noted, and relocation assistance may be available for qualified candidates. The role requires prolonged standing, frequent travel, bending or stooping, communication, auditory and visual skills, attention to detail, legible writing, and the ability to lift, push, or pull 20–50 pounds. Exposure to chemicals, blood or body fluids, and infectious diseases may occur.
