About The Position

21st Century Home Health Services (21HHS) is seeking a driven Regional Quality Manager focused on improving patient care, strengthening clinical practice, and supporting the team. This role involves setting care delivery standards across a region, building effective systems, identifying areas for improvement, coaching teams, and fostering a culture where quality is a shared responsibility. The position requires onsite presence at one of the local offices (Walnut Creek, San Jose, Burlingame, Sacramento) and is not a remote opportunity.

Requirements

2+ years in home health or healthcare operations is highly preferred
Multi-site or regional support experience is a plus
Outstanding Customer Service Skills

Nice To Haves

Highly Preferred California Clinical License (Nursing, PT, ST, OT)
Bachelor's degree in healthcare administration, Business, Nursing, or related is a plus

Responsibilities

Own clinical quality across the region by monitoring care delivery through chart audits, field observations, and outcome data, and translating findings into concrete practice changes.
Track key metrics such as patient outcomes, hospitalization and readmission rates, infection rates, patient satisfaction, star ratings, and clinical documentation accuracy, and translate trends into action.
Participate in performance improvement projects from problem identification through root cause analysis, intervention design, and measurement of results.
Standardize best practices across branches, replacing workarounds with clear, repeatable processes for clinicians.
Coach and develop clinical teams through real-time feedback, competency assessments, and targeted education.
Investigate incidents, complaints, and adverse events with a systems lens, identifying root causes and implementing safeguards.
Partner with regional and operational leadership to align quality goals with staffing, scheduling, and growth plans.
Prepare the region for surveys and accreditation reviews through ongoing readiness and continuous audit-readiness.
Build and maintain the region's quality infrastructure, including dashboards, audit tools, reporting cadences, and QAPI committee structure.
Champion a culture of accountability and psychological safety where staff report concerns early and near-misses are learning opportunities.
Lead the regional quality function, setting priorities and holding a consistent standard for quality.
Serve as a working partner to regional and site leadership, participating in operations meetings and building realistic improvement plans.
Develop clinical leaders and supervisors in coaching, documentation review, and case escalation.
Onboard and orient new clinical staff to regional standards of practice and identify onboarding process gaps.
Manage the region's education and training calendar, prioritizing topics based on audit findings, incident trends, and clinician feedback.
Contribute to hiring and performance decisions for clinical roles, applying a quality lens to competency evaluation, corrective action, and retention.
Support operational readiness during growth, transitions, and acquisitions.
Report regional performance to senior leadership, detailing improvements, challenges, and resource needs.
Own the patient and family experience as a quality measure, resolving concerns and using complaint patterns diagnostically.
Build trust with customers by being a reliable point of contact for clinical questions or concerns.
Communicate quality outcomes to external partners using performance data to demonstrate the region's value.
Represent the region in the community at partner meetings, clinical in-services, and industry events.
Coach staff on service recovery and patient communication.
Partner with sales and business development to ensure growth commitments align with clinical capacity.
Translate patient, family, and partner insight into process change.
