About The Position

Home Health Care, Inc. is seeking a Regional Physical Therapist (PT) to serve patients throughout Southern and Southwest Minnesota. This role offers the freedom to think critically, the flexibility to manage your own day, and the satisfaction of seeing patients make meaningful progress where they live. As a home health therapist, you will have the opportunity to practice at the top of your license with the autonomy to make clinical decisions, treating one patient at a time without the distractions of a busy outpatient schedule. Requirements

• Active Minnesota Physical Therapist (PT) license.
• Strong clinical reasoning and patient-centered approach.
• Excellent communication and organizational skills.
• Comfortable working independently across a regional territory.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate and treat patients throughout Southern and Southwest Minnesota in their homes.
• Design individualized treatment plans focused on restoring mobility and independence.
• Educate patients and caregivers to improve long-term outcomes.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the care team.
• Monitor progress and adjust treatment plans as patients improve.
• Complete timely and accurate documentation.
