Requirements

  • Active Minnesota Physical Therapist (PT) license.
  • Excellent clinical judgment and communication skills.
  • Passion for delivering high-quality care in community settings.
  • Ability to work independently while managing a regional territory.
  • Valid driver's license and reliable transportation.

Responsibilities

  • Evaluate patients throughout Southern and Southwest Minnesota in their homes.
  • Develop individualized treatment plans that promote mobility, strength, balance, and independence.
  • Recommend home safety modifications and adaptive equipment.
  • Educate patients and caregivers to support long-term success.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and the interdisciplinary care team.
  • Complete timely, accurate documentation and monitor patient progress.
