Requirements
- Active Minnesota Physical Therapist (PT) license.
- Excellent clinical judgment and communication skills.
- Passion for delivering high-quality care in community settings.
- Ability to work independently while managing a regional territory.
- Valid driver's license and reliable transportation.

Responsibilities
- Evaluate patients throughout Southern and Southwest Minnesota in their homes.
- Develop individualized treatment plans that promote mobility, strength, balance, and independence.
- Recommend home safety modifications and adaptive equipment.
- Educate patients and caregivers to support long-term success.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and the interdisciplinary care team.
- Complete timely, accurate documentation and monitor patient progress.
