About The Position

Valir PACE serves older adults who meet eligibility requirements for the Program of All-Inclusive Care for the Elderly (PACE). Through an interdisciplinary team approach, we provide comprehensive medical and supportive services designed to help participants live as independently as possible in their communities. Requirements

  • Graduate of an accredited Physical Therapy program
  • Current Oklahoma Physical Therapist license or eligibility for licensure
  • Current BLS certification
  • Valid driver's license and reliable transportation

Nice To Haves

  • Experience in outpatient rehabilitation, acute care, skilled nursing, home health, geriatrics, or PACE
  • Experience working with medically complex older adults
  • Strong communication and interdisciplinary collaboration skills
  • Passion for participant-centered care

Responsibilities

  • Perform comprehensive physical therapy evaluations and reassessments
  • Develop individualized treatment plans focused on mobility, balance, strength, endurance, and functional independence
  • Provide therapeutic interventions that support participant safety and mobility
  • Participate in interdisciplinary care planning meetings and case conferences
  • Conduct home visits, when appropriate, to assess mobility, safety, and functional performance
  • Recommend adaptive equipment and environmental modifications
  • Educate participants, caregivers, and team members on therapeutic interventions and safety strategies
  • Maintain timely, accurate clinical documentation
  • Collaborate with the Interdisciplinary Team to provide participant-centered care
