The Refugee Navigator will provide non-clinical interventions to engage patients in their care and connect them to community support. This position is responsible for serving as direct support to recently resettled patients in screening for, understanding, and making appropriate referrals for basic social needs (e.g., transportation, food, and housing) that directly impact the patient's short- and long-term health. The Refugee Navigator will serve as the liaison and point of contact for the resettlement agencies and the Nebraska Refugee Coordinator. This position will be an integrated part of the CHI clinic care team and will work directly with patients, and build and maintain relationships with community resource partners. Participate as a full member of the clinic team, including attending team meetings and care conferences as appropriate. Act as peer support and advocate for recently resettled refugee patients as they navigate the medical and social service system. Work with refugee patients and families on needs that may or will affect the patient’s health, including transportation issues, housing, financial concerns, etc. Identify and link families to community resources and services, assisting with coordination as necessary. Assist in the coordination and completion of necessary citizenship paperwork, including N-648 disability waivers. Enter and maintain electronic health records, and other program documentation in a timely manner.
