About The Position

The Referral Specialist is responsible for coordinating and completing referrals ordered by health center providers, acquiring and verifying third party information for referrals, obtaining and arranging referral appointments, tracking referrals, and collecting consult reports from external specialists. This role works in collaboration with the Patient Care Coordinator to ensure duties are completed. The Referral Specialist will coordinate all aspects of referrals ordered by health center providers on behalf of patients, ensuring the client is PCP aligned before working on the referral. They will enter appropriate structured data fields in the referral tracking module in the electronic medical record and maintain communication with the provider and patient regarding the most appropriate specialist to refer to in each case. The position also involves ensuring proper paperwork is given to patients for appointments per orders (i.e., labs, x-rays, progress notes), tracking referral appointment outcomes in the medical record, and bringing them to the attention of the ordering provider. The Referral Specialist will assign patients to securing their own appointments based on their ability and need for support, secure insurance authorization as needed, and obtain consult reports from specialty providers to log in the medical record. They will maintain a specialty provider list on a shared drive, including payor sources they accept, and participate in daily morning huddles, providing updates on the status of referrals for clients being seen on today’s visits. The role may require providing coverage at alternate locations and working occasional evenings and weekends as needed. Requirements

  • High school diploma or equivalent
  • One year of experience in a medical related field preferred.
  • Interest in and ability to work with culturally diverse populations.
  • Skilled in interpersonal relationships with a culturally diverse population.
  • Ability to work occasional evenings and weekends with advance notice.
  • Strong attention to detail and organization.
  • Ability to prioritize multiple demands at once.
  • Experienced with Microsoft Office (Word, Excel), Outlook, and the Internet.
  • Must possess a high level of interpersonal skills to handle sensitive and confidential client information and situations.
  • Must be willing to participate in advocacy events.

Responsibilities
