About The Position
Behavioral Health Connections, Inc. is a provider of patient-focused, comprehensive mental health services. We offer mental health evaluations to children, adolescents, adults and older adults in crisis 24 hours a day, 7 days a week response, including all holidays. Our service model focuses on the assessment, diagnosis, treatment, and disposition of your mental health patients. Based on the results of an assessment, we determine the most appropriate level of care and recommend treatment facilities. The tele-psychiatric assessment and consultation services we offer are designed to connect people with the care provided best equipped to address their mental health issues. Rapid response means that the patient will get to the most appropriate level of care faster and expedite their journey to wellness. Behavioral Health Connections (BHC) in Dunwoody, ATL is seeking a dynamic and talented Referral Management Specialist PRN Varied shifts that serves to assess and coordinate the care of patients referred for Mental Health Services. They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs.
Requirements
- Masters Degree in Social Work, Psychology, Counseling, Behavioral Health or related field.
- Current CPR, Vaccines, if required by a customer hospital.
- Must have a valid driver’s license.
Nice To Haves
- Preferred Licensed in state, LMSW, LCSW, LPC
- Preferred 1-3 years work related experience
Responsibilities
- Review assessment referrals received from various sources in collaboration with the RN and other clinicians, to assess patient needs.
- Assist in coordinating patient care.
- Responsible for service coordination by matching patients with appropriate mental health services and providers within UHS, considering factors such as specialty, location, systems complexities and insurance coverage.
- Maintain patient records, update databases, and ensure accurate documentation of patient information in approved EMR and other platforms.
