About The Position

The Referral Coordinator is responsible for managing patient referrals, ensuring accurate clinical information is provided to specialists, and maintaining ongoing tracking and documentation of referrals. This role serves as a point of contact for patients and specialists, assisting with any questions or concerns related to referrals, including potential issues with language, social barriers, or transportation. The coordinator also ensures complete and accurate patient registration and maintains patient records. Additionally, the role involves collaborating with physician and clerical staff, assisting with check-in/out processes, and providing backup for Medical Office Specialists. The position also includes setting up and explaining medication cards to patients.

Requirements

  • High School Diploma or equivalent required.
  • Current Medical Assistant Certification and/or Registration required through an accredited organization (i.e. AMT, AAMA, NHA, etc.)
  • Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.
  • Speaks clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions.

Nice To Haves

  • 1-2 years related experience preferred.

Responsibilities

  • Assemble information concerning patient's clinical background and referral needs; per referral guidelines, provide appropriate clinical information to specialists; review details and expectations about the referral with patients.
  • Prioritize referrals by their urgency and address them in a timely manner; maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety.
  • Participate in the selection, orientation and training of referral processes.
  • Review details and expectations about the referral with both ordering providers and patients; request new referrals to be ordered when applicable.
  • Ensure complete and accurate registration, including patient demographic and current insurance information.
  • Serve as the point of contact for patients and specialists for any questions or rising concerns.
  • Assist in problem-solving potential issues related to their referral due to language or social barriers, and transportation. If unable to help, assist in finding the right point of contact to further help.
  • Accurately and truthfully complete patient records for the referral process in a timely manner.
  • Work harmoniously with Physician and Clerical staff to meet patient needs; assist Medical Office Specialists with check-in/out process; serve as backup for Medical Office Specialists vacations, illnesses, lunches, etc.
  • Complete daily routine assignments.
  • Manage difficult or emotional customer/patient situations; respond promptly to inquiries, respond to requests for service and assistance; meet commitments.
  • Set-up medication cards, medication card explanation/patient education pertaining to medication card.
