About The Position

Under general supervision, performs work of moderate difficulty coordinating public programs. Pathways to Community is a collaborative partnership between the Coconino County Sheriff’s Office and Coconino County Health and Human Services (CCHHS) that provides reentry support for individuals leaving the Coconino County Jail. Individuals released from custody receive immediate connection to social, behavioral, housing, transportation, and legal services to support their successful transition back into the community. The Reentry Coordinator is responsible for coordinating comprehensive case management and reentry navigation services for individuals who are currently incarcerated or recently released. This role works closely with on-site partners, community service providers, and clients’ legal teams to ensure seamless support, resource connection, and continuity of care throughout the reentry process. This position is funded by Pathways Legislative SB1735 through 06/30/2027.

Requirements

  • Associate degree in public/business administration, or a related field within subject area, and two years of administrative experience; OR Three years of administrative experience; OR Any equivalent combination of education, training and experience which demonstrates the ability to perform the duties of the position.
  • Must obtain an Arizona driver's license by date of hire and maintain it throughout employment.
  • If this position is in Community Services, then a fingerprint clearance card is required.
  • Experience respectfully working with vulnerable populations, including individuals experiencing serious mental illness, homelessness, behavioral health challenges, and those who are engaged with the criminal justice system.
  • Familiarity with the challenges that individuals leaving incarceration face.

Responsibilities
