Under general supervision, performs work of moderate difficulty coordinating public programs. Pathways to Community is a collaborative partnership between the Coconino County Sheriff’s Office and Coconino County Health and Human Services (CCHHS) that provides reentry support for individuals leaving the Coconino County Jail. Individuals released from custody receive immediate connection to social, behavioral, housing, transportation, and legal services to support their successful transition back into the community. The Reentry Coordinator is responsible for coordinating comprehensive case management and reentry navigation services for individuals who are currently incarcerated or recently released. This role works closely with on-site partners, community service providers, and clients’ legal teams to ensure seamless support, resource connection, and continuity of care throughout the reentry process. This position is funded by Pathways Legislative SB1735 through 06/30/2027.
