About The Position

Leaving jail can mean navigating housing, employment, health care, treatment, benefits, transportation, and other basic needs—all at once. As a Reentry Case Manager with Bridgeways’ Jail Transition Program, you’ll work with adults preparing to return to the community and after release, helping them identify needs, connect with resources, and work toward greater stability. This is a community-based role for someone who enjoys working independently, building relationships, solving problems, and turning “this person needs help” into “here’s our next step.”

Requirements

  • Experience providing case management or direct support to people navigating complex needs.
  • Comfortable meeting new people and building rapport with individuals from a wide range of backgrounds and circumstances.
  • Comfortable working within jail and criminal justice settings.
  • Ability to manage multiple clients, appointments, deadlines, and follow-ups without needing someone else to continually direct your day.
  • Proactive about connecting people with resources—willing to make the connection, follow up, troubleshoot barriers, and help determine the next step.
  • Basic computer skills and comfortable communicating by email, phone, and in person.
  • Able to travel to required work locations throughout Snohomish County.
  • Able to meet applicable background and jail-access requirements.
  • Authorized to work in the United States.
  • Consideration given to the combination of education, professional experience, volunteer experience, and lived experience that has prepared you to provide effective case management and work with people experiencing complex barriers.
  • Relevant education may include social work, psychology, criminal justice, human services, or a related field.

Nice To Haves

  • Experience with reentry, behavioral health, substance use recovery, housing or homelessness services, outreach, benefits navigation, criminal justice, or community-based social services is especially helpful.
  • Certifications such as Certified Case Manager, Mental Health First Aid, or Peer Counselor are welcomed but not required.

Responsibilities

  • Independently manage a caseload, typically 20-30 clients, with support from your supervisor and team.
  • Meet with clients in local jails and throughout Snohomish County to assess needs and plan for successful reentry.
  • Connect clients with housing, behavioral health and substance use treatment, health care, employment, benefits, transportation, and other community resources.
  • Develop transition plans, monitor progress, troubleshoot barriers, and follow up to keep things moving.
  • Build relationships and coordinate with jail staff, community providers, government agencies, and other partners.
  • Advocate for clients and provide support when challenges arise.
  • Maintain timely case notes and documentation in the electronic medical record (EMR).
  • Manage your own schedule, appointments, priorities, and follow-ups.

Benefits

  • Up to 31 paid days off for new employees: 15 flexible-use PTO days, 10 paid holidays, 2 personal days, and a paid year-end closure.
  • Comprehensive medical, dental, and vision coverage, with medical premiums starting as low as $39 biweekly for eligible employees.
  • Optional Life, Critical Illness, Accident, and pet insurance.
  • 401(k) with immediate vesting, plus an employer contribution equal to 3% of gross wages after one year of service.
  • Mental health coverage through Teladoc Mental Health, including access to therapists and psychiatrists.
  • Roadside Assistance for all employees.
  • Long-term disability for eligible employees.
  • Vision, prescription, and entertainment discount programs.
