About The Position
Gándara Mental Health Center is hiring a court-based Recovery Support Navigator in Brockton, Massachusetts. The role provides case management services focused on participant engagement, exploring services, coordinating referrals, and maintaining short-term continuity of care. The navigator works with providers, family members, and community resources to support improved functioning and access to and sustained use of needed services through a flexible, comprehensive service delivery model. Bilingual candidates are encouraged to apply.
Requirements
- Minimum bachelor’s degree in psychology or equivalent related work experience.
- At least one year of related experience.
- Understand the population served and the information-sensitivity protocols required in mental health, including HIPAA and privacy laws and regulations.
Nice To Haves
- Bilingual Spanish/English skills preferred.
Responsibilities
- Assess clients’ non-medical and mental health needs.
- Develop, implement, and monitor service plans consistent with identified needs.
- Communicate effectively and promptly with clients and collateral contacts.
- Coordinate with internal departments, in conjunction with or under the direction of a supervisor, to support optimal use of agency services.
- Attend internal and external meetings relevant to individual clients or CSP services.
- Attend relevant training as required by the supervisor.
