About The Position

Gándara Mental Health Center is seeking a court-based Recovery Support Navigator to provide case management services, including participant engagement, service exploration, referral coordination, and short-term continuity of care. The role works with providers, family members, and community resources to support improved functioning and access to, and sustained use of, needed services through a flexible and comprehensive service delivery model. The position is based in Brockton, Massachusetts. Bilingual candidates are encouraged to apply.

Requirements

• Minimum bachelor’s degree in psychology or equivalent related work experience.
• Understand the population and the information-sensitivity protocols required in mental health, including HIPAA and privacy laws and regulations.
• At least 1 year of related experience.

Responsibilities

• Assess clients’ non-medical and mental health needs.
• Develop, implement, and monitor service plans that address identified needs.
• Communicate effectively and promptly with clients and collaterals.
• Coordinate internally with departments, in conjunction with or under the direction of a supervisor, to support optimal use of agency services.
• Attend external and internal meetings relevant to individual clients and/or CSP services.
• Attend relevant training as required by the supervisor.
