About The Position

Mat-Su Health Services (MSHS) is seeking a compassionate and driven Recovery Outreach Case Manager to join their integrated care team. This role is part of the Positive Pathways Program and focuses on substance use disorder recovery initiatives. The position involves community outreach, building rapport with individuals hesitant to engage in treatment, and assisting them in accessing various resources such as medical, behavioral health, recovery support, housing, and employment. The Case Manager will utilize harm reduction principles, motivational interviewing, and strengths-based engagement strategies. They will serve as a key point of contact for participants, providing follow-up services, facilitating referrals, supporting Wellness Recovery Action Plans (WRAPs), assisting with benefit applications, and promoting continued engagement in treatment and recovery services. The role also supports participant engagement through recovery support groups and community-based outreach activities aimed at reducing barriers to care and improving health outcomes. This is not a desk job and requires a self-starter who enjoys meeting people in various community settings.

Requirements

  • BA/BS degree preferred in Social Work, Psychology, Human Services, or related field with some work experience.
  • 5 years of relevant work experience may be substituted for education.
  • Education & relevant work experience equivalent to 5 years may be considered.
  • Demonstrated ability to communicate effectively, both orally and in writing.
  • Ability to independently plan, schedule and conduct program activities.
  • Knowledge of standard office equipment including personal computer and standard word-processing programs experience.
  • General Clear background check (subject to DHSS fingerprinted background check).
  • Current driver’s license; clear driving record.
  • Current tuberculosis screening test result.
  • Current First Aid and CPR certifications.
  • CPI (Nonviolent Crisis Intervention) certifications must be obtained when offered through the agency; may substitute current MANDT certificate for CPI.

Responsibilities

  • Serves as the primary outreach and engagement specialist for Mat-Su Health Services' Positive Pathways Program and related substance use disorder recovery initiatives.
  • Provides trauma-informed, recovery-oriented case management services to individuals affected by substance use disorders and related social determinants of health.
  • Conducts community outreach and establishes rapport with individuals who may be hesitant to engage in treatment.
  • Assists participants in accessing medical, behavioral health, recovery support, housing, employment, and other community resources.
  • Utilizes harm reduction principles, motivational interviewing, and strengths-based engagement strategies to support participants at all stages of change.
  • Serves as a key point of contact for participants enrolled in the Positive Pathways Program.
  • Provides follow-up services, facilitates referrals, supports development of Wellness Recovery Action Plans (WRAPs), assists with benefit applications and resource navigation, and promotes continued engagement in treatment and recovery services.
  • Supports participant engagement through recovery support groups and community-based outreach activities designed to reduce barriers to care and improve health outcomes.
  • Maintains regular contact with psychiatric hospitals, jails, shelters, and other locations to identify and offer services and assist qualifying clients/families to obtain benefits and connect to services.
  • Facilitates participant engagement before, during, and after recovery support groups by assisting with enrollment, resource navigation, referrals, benefit applications, appointment scheduling, and follow-up services.
  • Provides education regarding overdose prevention, naloxone administration, fentanyl test strips, safer substance use practices, and prevention of HIV, Hepatitis C, and other communicable diseases.
  • Maintains accurate and timely documentation in the Electronic Health Record (EHR) and any required data tracking tools within the established time frame.
  • Participates in treatment planning activities and monitors client progress toward identified recovery, treatment, and wellness goals in accordance with Division of Behavioral Health regulations and agency standards.
  • Participates in interdisciplinary case reviews, quality improvement activities, care coordination meetings, and crisis response planning as appropriate to support client engagement and continuity of care.
  • Conducts field-based and on-site outreach to high-risk populations, including individuals experiencing homelessness, substance use disorders, and justice involvement, in coordination with the SDOH Care Manager.
  • Builds trust with individuals through motivational interviewing, harm reduction education, and culturally responsive engagement.
  • Provides linkage to care, including MOUD, primary care, behavioral health, housing, and recovery services.
  • Supports participants in developing Wellness Recovery Action Plans (WRAPs) and conducts follow-up contact within 72 hours of enrollment or referral to establish rapport, assess immediate needs, provide education, and facilitate connection to services.
  • Tracks service utilization, client engagement, and health outcomes within the Positive Pathways grant requirements.
  • Participates in biweekly interdisciplinary team meetings and contributes to collaborative treatment planning.
  • Assists clients accessing public benefits and non-clinical supports, including harm reduction supplies, transportation, hygiene, and food resources.
  • Participates in professional development, including harm reduction and motivational interviewing training.
  • Provides education about mental health and services to outside agencies and organizations as requested.
  • Actively works to assure implementation of the values and principles of psychosocial rehabilitation.
