About The Position

HealthRIGHT 360 is a nonprofit organization dedicated to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to address systemic barriers to healthcare and promote health equity. We offer a range of services including mental health care, substance use treatment (residential and outpatient), and primary health services. We also provide transitional support for individuals re-entering the community from the criminal justice system. By integrating physical and behavioral health, we aim to empower individuals to overcome challenges, foster resilience, and facilitate recovery. The 214 Men’s Residential Step Down (MRSD) is a program of HealthRIGHT 360, located in San Francisco's Alamo Square neighborhood. It is a 64-bed transitional housing facility for men who have completed at least 30 days of residential substance use disorder treatment. MRSD offers temporary housing to support clients in their next stage of sobriety. The On-Call Recovery Coach plays a crucial role in assessing client needs and strengths to support their health and recovery journey. Through individual coaching, adherence to program-specific requirements, and case management, On-Call Recovery Coaches help clients navigate systems of care while fostering a supportive recovery environment. They are responsible for maintaining the safety and security of the program and its residents through regular house checks and providing a supportive atmosphere. Effective communication is key, as they relay important information to the interdisciplinary team and perform crisis intervention when necessary. On-Call Recovery Coaches ensure the facility environment is safe and welcoming for all residents.

Requirements

  • High School Diploma or equivalent.
  • Valid California Driver’s License and automobile insurance.
  • Experience working successfully with diverse populations specifically with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.
  • Knowledge of gender-responsive, trauma informed, and co-occurring treatment.
  • Strong computer skills, especially with Microsoft Office applications and internet applications.
  • Ability to work within a frequently changing project scope while maintaining overall direction and structured priorities.
  • Self-motivated, dependable, creative, and proactive approach to work; understands importance of working independently and within a team environment.
  • Ability to enter data into various electronic systems while maintaining integrity and accuracy of data.
  • Excellent verbal and written communication skills and interpersonal skills.
  • Ability to handle sensitive information in a confidential manner.
  • Qualified candidates with arrest and conviction records will be considered for employment.
  • Must be capable of obtaining and maintaining a satisfactory background check.

Responsibilities

  • Provides direct services such as intakes, assessment, crisis intervention, and case management as needed.
  • Responsible for client retention and completion, facilitates individual coaching sessions with each caseload client.
  • Provides learning experience opportunities and offers support to assist clients in meeting their long-term recovery goals.
  • Proactively links clients to both internal and external resources based on their recovery needs and follows up on their progress/status.
  • Performs house runs to ensure and maintain accountability of all clients and the safety and security of the facility.
  • Performs crisis intervention and communicates with the treatment team as unforeseen situations arise.
  • Documents client updates and incidents in the facility log daily.
  • Performs CPR and First Aid as needed.
  • Works weekends and holidays as needed.
  • Facilitates community meetings and ensures compliance with facility rules.
  • Documents all client updates and facility incidents daily in the facility log.
  • Performs periodic house runs to ensure and maintain the safety and security of the facility.
  • When assigned the DOT, documents and accurately distributes client money, ensures client medications are securely stored and properly accounted for, and holds facility keys and floater cellular phones.
  • As needed, accompanies clients to offsite appointments related to their recovery.
  • Attends and completes all required trainings and meetings as assigned by the facility manager.
  • Plans and facilitates client celebrations and special events.
  • Collaborates with each client and other available internal and external resources to develop transition plans, progress notes, and appropriate updates in support of the health and recovery needs of the client.
  • Properly documents all individual encounters and completes the discharge paperwork/process and required agency assessments in a timely manner.
  • Maintains accurate records by data entering documentation into various electronic systems for all clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
  • Conducts regular and vigilant monitoring of clients throughout the night to ensure their safety and well-being.
  • Utilizes observation and assessment skills to detect any changes in clients' behavior or condition during the night.
  • Responds promptly and effectively to any emergencies that arise during the overnight shift, including medical, psychiatric, and safety incidents.
  • Accurately documents any incidents, interventions, and observations made during the night.
  • Prepares detailed reports for the day shift staff to ensure continuity of care and communication.
  • Develops individualized recovery plans based on assessment findings.
  • Tracks client progress and follows up on recovery goals.
  • Communicates relevant information to interdisciplinary team members.
  • Addresses safety concerns and implements necessary measures promptly.
  • Fosters a supportive and welcoming environment for clients.
  • Manages multiple tasks, such as intakes, assessments, and documentation.
  • Prioritizes tasks and maintains accurate records within required timelines.
  • Engages clients in recovery activities and promotes retention and completion of treatment.
  • Plans and facilitates client celebrations and special events to enhance the recovery experience.
  • Organizes activities that promote a positive and supportive community atmosphere.
  • Performs other duties as assigned.
