About The Position

Carefor is seeking a Rapid Response Nurse (RPN) to join their atHome bundled care programs. Reporting to the Manager of Transition Programs, this role is responsible for the provision and management of quality nursing care to clients transitioning out of hospital. The RPN will manage the wrap-around care coordinated and delivered to clients, with support from the care team. Key duties include completing InterRAI-HC assessments, developing care plans, collaborating with various healthcare professionals (social workers, PTs, OTs, etc.), and delegating tasks to personal support workers. The role emphasizes proactive care planning using assessment findings to minimize emergency department visits by supporting clients in their homes. The RPN also provides clinical support, addresses PSW job performance issues, participates in case conferences, offers training, and provides performance feedback. As a resource to the Transition Program, the RPN supports the care team and clients by modeling practice standards, defining care processes, and collaborating on PSW education. The position also supports the implementation of evidence-based practice projects for Carefor’s Transition Programs.

Requirements

  • Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario is required.
  • Three (3) years recent experience of working within the adult home health care sector with related experience to support the provision of restorative care to older adults.
  • Knowledge of the related regulatory and legislative requirements within Ontario is required.
  • Proficiency in using Microsoft Office applications (e.g., Outlook, Word, Excel, etc.).

Nice To Haves

  • Current Standard First Aid Certificate preferred
  • Current CPR level C Certificate preferred
  • Up to date immunizations for Measles, Mumps, Rubella, Varicella, Hepatitis B, Tetanus and Diphtheria preferred
  • Certification in clinical specialties and chronic disease management (e.g., GPNC(C) Gerontology Practical Nurse Certified (C) Canada or CMSN(C) Medical-Surgical Nursing (C) Canada) is an asset.
  • Ability to promote and facilitate evidence-based practice within a community setting with a demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams.
  • Superior communication skills and ability to communicate effectively to direct and motivate others while maintaining a persuasive and credible presentation style at all levels of the organization.
  • Demonstrated effective analytical, and problem-resolution skills.
  • Excellent organizational, planning and time management skills.
  • Experience in a unionized environment is an asset.
  • Certified RAI Assessor is preferred
  • Active member of RPNAO is an asset.
  • Bilingualism (French/English) is an asset.

Responsibilities

  • Supports the care management and home care assessment responsibilities of clients transitioning out of hospital within the atHome Program
  • Delivers nursing care to clients within the atHome Program
  • Acts as a resource to the atHome Program care team, providing coaching, education, and instruction to encourage individual and operational performance objectives. Encourages and models evidence-informed practice
  • Supports the performance management process with assigned staff and engages with appropriate managers regarding any performance issues
  • Contributes to human resources management including employee relations, health and safety, continuing education, performance development, recognition of staff and orientation, as required
  • Assists the Manager in identifying areas for recruitment, anticipates and monitors staffing needs and initiates appropriate action to obtain/maintain an adequate compliment staff for the atHome program
  • Investigates and evaluates client and staff concerns/complaints and implements appropriate resolutions, as required
