Nurse Practitioner PRN- Rapid Response

CHP Health•San Diego, CA
•$90 - $105

About The Position

Correctional Healthcare Partners (CHP Health) provides medical, mental health, and dental care in jails and detention facilities in California and Washington. The Nurse Practitioner PRN Rapid Response Provider serves as the first clinical safety net for patients at the San Diego Adult Detention Facility during assigned 24-hour periods. The provider connects with on-site nursing staff by telehealth using a laptop or tablet to assess, treat, and stabilize patients. Scheduled telehealth visits and documentation may be completed between calls, while the provider remains available for urgent requests throughout the assigned period. The provider assesses newly incarcerated patients and initiates stabilization plans within 24 hours of arrival, including prescribing medications for medical and psychiatric stabilization and providing immediate mental health support when needed. The role also includes telehealth visits for routine, urgent, and emergent conditions; ordering medications, labs, and referrals; completing clinical approvals; and coordinating care with nursing and mental health staff. The provider completes all encounter documentation and EHR orders, reviews assigned lab and diagnostic results and patients returning from outside care, supports chronic care and continuity of care, and communicates patient-safety or contract-performance concerns to the Medical Director. Rapid response days are set in advance, and the provider is available for the full 24 hours on those days. Supplemental work outside regular rapid response days may include in-person facility coverage or an additional rapid response day. The role is classified as non-exempt and offers compensation of $90–$105 per hour.
